  • Understand the benefits and risks of the treatment proposed by your provider and be involved in decisions about your health and treatment.
  • Ask for a change of provider.
  • Refuse any treatment or service.
  • Receive treatment in a language you understand.
  • Privacy, including safeguarding your medical records.
  • Receive a copy of your medical records and details of your bill.

Your Responsibilities

  • Share accurate information about yourself and your health, including past medical history and medications.
  • Use the website at anagen.xyz and associated services for communication. Notify us if you need help using these tools.
  • Ask for help if you don’t understand your treatment and follow your treatment plans. Failure to comply with treatment plans may lead to termination of care.
  • Inform us of your insurance coverage, including Medicare, Medicaid, or Medi-cal.
  • Show up for appointments or cancel them on time. Repeated no-shows or late cancellations may result in discontinuation of care.
  • Pay your Anagen LLC bills on time.
  • Treat your providers and staff with respect to maintain a safe and trusting environment.
  • Do not record medical visits without permission. If you wish to record a visit, ask your provider first.
